What are your hours?

We are open Monday through Friday, 7:45 AM until 3:30 PM. Client drop off begins at 7:45 AM.

how do i refer a family member?

Please contact the Director, Jake Davis. You may also call the center at 469-758-0063.

how do you establish eligibility?

Eligibility is established through your local authority (Denton County MHMR, Dallas County Metrocare Services, or Collin County Health Services).

who is eligible?

Adults with intellectual and developmental disabilities who are diagnosed with IDD/IQ below 75 before the age of 18 or a related condition before the age of 22.

what is the enrollment process?

Please complete and submit the New Client Application. Our enrollment committee will notify you within 7 working days of their decision. If you receive Medicaid Waiver Program services, please notify your case manager of your interest in attending Caring Pathways of Carrollton. Once all paperwork is complete and approved, new clients may join as openings are available!

how do i volunteer?

are you a state agency?

No, we are not a state agency. We are a non-profit organization developed to serve the special needs population in Carrollton, Texas, and surrounding areas.

are you open on holidays?

We are closed on the following holidays:

New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day

In the case of inclement weather, we determine closings based on Carrollton-Farmers Branch ISD.

Texas Health and Human Services is the place to start. From there, contact the county office responsible for services for people with intellectual and developmental disabilities for the county in which you reside. (See Establishing Eligibility.)
